Streszczenie: Metoda MALDI-TOF MS jest nowym i coraz częściej wykorzystywanym w laboratoriach klinicznych narzędziem do identyfikacji drobnoustrojów. Szerokie zainteresowanie tą metodą wynika z jej wysokiej dokładności, szybkości uzyskiwania wyników identyfikacji mikroorganizmów oraz stosunkowo niskiego kosztu analiz. Implementacja tej techniki do identyfikacji dermatofitów jest jednak trudna. Trudności te spowodowane są naturalną złożonością biologiczną grzybów strzępkowych, bardzo wolnym tempem wzrostu drobnoustrojów i częstym wytwarzaniem pigmentów. Ponadto, identyfikacja dermatofitów tą techniką stanowi wyzwanie ze względu na brak jasnej definicji gatunku dla niektórych taksonów lub w obrębie niektórych kompleksów gatunkowych. Przegląd literatury naukowej wskazuje, że wiarygodność identyfikacji dermatofitów opartej na MALDI-TOF MS waha się między 13,5 a 100%. Liczne czynniki krytyczne związane z rutynowymi procedurami laboratoryjnymi, tj. rodzajem podłoża hodowlanego, czasem inkubacji, techniką ekstrakcji białek, typem urządzenia czy wersją biblioteki widm referencyjnych, warunkują taką zmienność w uzyskiwanych wynikach. Pomimo wielu ograniczeń metoda MALDI-TOF MS stanowi istotny postęp techniczny w diagnostyce mykologicznej i alternatywę dla czasochłonnej i pracochłonnej identyfikacji dermatofitów opartej o cechy morfologiczne oraz sekwencjonowanie DNA. Niemniej jednak zanim zostanie wdrożona do rutynowych badań diagnostycznych, konieczne jest rozszerzenie biblioteki widm referencyjnych dermatofitów, a także opracowanie procedur analizy bezpośredniej z próbek dermatologicznych.

Abstract: The MALDI-TOF MS method is a new technique, which is being increasingly used in clinical laboratories for identification of microorganisms. The wide interest in this method has been aroused by its high accuracy, instantaneous identification results, and relatively low cost of analyses. However, the application of this technique for identification of dermatophytes poses difficulties. They are caused by the natural biological complexity of filamentous fungi, very slow growth of cultures, and frequent production of pigments. Furthermore, identification of dermatophytes with this technique is a challenge due to the lack of a clear species definition for some taxa or within certain species complexes. A review of scientific literature indicates that the reliability of identification of dermatophytes based on MALDI-TOF MS is in the range between 13.5 and 100%. This variability is determined by many critical factors associated with routine laboratory procedures, i.e. the type of culture medium, incubation time, protein extraction technique, type of device, or version of the reference spectrum library. Despite these numerous limitations, the MALDI-TOF MS method is part of the significant technical progress in mycological diagnostics and an alternative to the time-consuming and labor-intensive identification of dermatophytes based on morphological traits and DNA sequencing. Nevertheless, before the technique can be implemented into routine diagnostic tests, it is necessary to expand the reference spectra library and develop procedures for direct analysis of dermatological samples.

Abstract: Among the Gram-positive anaerobic bacteria associated with clinical infections, the Gram-positive anaerobic cocci (GPAC) are the most predominant and account for approximately 25–30% of all isolated anaerobic bacteria from clinical specimens. They can be cultured from a wide variety of sites, particularly abscesses and infections of the mouth, skin and soft tissues, bone and joints and upper respiratory and female genital tracts. Most infections involving GPAC are polymicrobial. Still, routine culture and identification of these slowly growing anaerobes to the species level has been limited in the diagnostic laboratory, mainly due to the requirement of prolonged incubation times and time-consuming phenotypic identification. The development of molecular methods such as PCR, multiplex PCR, sequencing of the 16S rRNA gene have led to improved identification of GPAC. In recent years, MALDI-TOF MS has been implemented in routine laboratories and utilized as a completely new approach for the identification of bacteria. Data from molecular methods have led to extensive taxonomic changes during the last decades and also to the occurrence of new genera and species. This review describes clinical significance of GPAC and virulence factors one of the most pathogenic species – Finegoldia magna.
